If this is your first HSE medical examination you will need to fill in a questionnaire and ask your general practitioner to verify your answers. A copy of the questionnaire can be found in Annex A of this HSE document
For your medical please bring
1. picture ID
2. sports gear for a fitness test (a timed step up test)
3. records relevant to your health and previous diving experience, if any
4. if this is your first medical, a completed medical questionnaire certified by your GP (this can be sent to the hyperbaric unit prior to the appointment so that the doctor can make sure that there are no problems which might restrict you from taking part in the type of diving you want to do)
5. if this is not an initial medical, the MA2 form from your last medical
Cost for the medical is £100 plus £17.50 VAT plus £30 for an audiogram (hearing test, required for the initial medical but only at subsequent medicals if there has been an ear problem)
Other occupational medicine referrals
Several of the doctors at the hyperbaric medicine unit are occupational medicine specialists and referrals can be arranged most weekdays. Cost will depend on the nature of the referral, length of consultation and any extra work required, such as report writing.
